People sometimes need time away from work, and to be able to excuse themselves from their absences or to be able to file sick leaves with pay, they need to submit a doctor\'s excuse. These notes are usually submitted written on the doctor's prescription pad or on a letterhead and signed by the attending physician. No matter what the illness or the reason is for the absence, it is important that the doctor\'s excuse contains complete information to be valid, including information about the physician, date of examination, name of patient and the medical certification.
